The Routine Vision Care Network and LASIK Network was established by the State Employees’
Insurance Board to provide our members with a discounted cost of obtaining routine
vision care. This IS NOT a Blue Cross vision network. It is your responsibility
to verify participation of a vision care provider by checking with his or her office
before you receive services.
Both directories contain our most current listings of providers participating in
the State of Alabama’s Routine Vision Care Provider Network and Laser Vision Correction
Network (LASIK). We have also included the provider’s name, address, and telephone
number.
This directory is arranged to help you quickly find a Routine Vision Care provider
in your area. Routine Vision Care participants are listed first by county, then
name, address, city, state, and contact number. The discounted costs are:
Member Payment Responsibility
Routine vision examination $40.00
Routine vision examination – with dilation $45.00
Initial contact lens fitting $25.00
Follow-up contact lens visit $25.00
Routine vision examination discounts, with or without dilation, are limited to one
per year.

All services listed require that you make your payment directly to the provider.
No claims are to be filed through Blue Cross Blue Shield or the State Employees’
Insurance Board.
Vision care services associated with a medical diagnosis (cataracts or glaucoma,
for example) should be filed with Blue Cross Blue Shield of Alabama. These benefits
will be paid in accordance with the State Employees’ Insurance Board’s Benefits
Plan Provisions.
