The SEIB has selected UnitedHealthcare® (UHC) to offer a custom UnitedHealthcare® Group Medicare Advantage (PPO) plan for all eligible retirees and their Medicare-eligible dependents. This plan is a custom plan, designed exclusively for SEIB retirees and should not be confused with Individual UnitedHealthcare (UHC) Medicare Advantage plans. Under this plan, you will have an enhanced benefit package, which includes fixed copays and lower out-of-pocket costs.

As a UHC Medicare Advantage member, your plan will help give you value for your health care dollar, offering benefits and services beyond what you will find with Original Medicare (Parts A and B). You will have a team committed to understanding your needs, connecting you to the care you need and helping you manage your health. UHC will link you to health and wellness resources and even schedule your wellness visits, including arranging an annual visit.

UnitedHealthcare® HouseCalls1

An in-home visit designed to complement your doctor’s care. A licensed and knowledgeable health care practitioner will review your health history and current medications, perform a health screening, identify health risks and provide health education.

Telephonic Nurse Support2

Health questions can come any time. Telephonic Nurse Support provides you 24/7 access to a registered nurse who can help you with sudden health concerns as well as:

  • Providing guidance for symptoms and directing you to the appropriate level of care
  • Questions about a medication
  • Understanding an ongoing health condition or new diagnosis

Renew Rewards

An in-home visit designed to complement your doctor’s care. A licensed and knowledgeable health care practitioner will review your health history and current medications, perform a health screening, identify health risks and provide health education.

Renew Active® by UnitedHealthcare3

Renew Active is the gold standard in Medicare fitness programs for body and mind, available at no additional cost. You’ll receive a free gym membership with access to the largest Medicare fitness network of gyms and fitness locations. This includes access to many premium gyms, on-demand digital workout videos and live streaming classes, social activities and access to an online Fitbit® Community for Renew Active and access to an online brain health program from AARP® Staying Sharp® (no Fitbit device is needed).

Rally CoachTM Programs

Rally Coach programs will help you start living a healthier, happier life with:

  • Real Appeal Weight Loss and Real Appeal Diabetes Prevention, online weight-loss programs designed to help you gain energy, reduce your risk of developing serious health conditions and achieve your long-term health goals
  • Wellness Coaching, an online and live coaching support program that provides access to a variety of digital health and wellness courses.
  • Quit For Life, a tobacco cessation program providing access to the tools and resources you need to help you quit all types of tobacco use.

Virtual Visits

See a doctor ($0 copay) or talk to a behavioral health specialist ($14 copay) anytime using live video chat from your computer, tablet or smartphone. With Virtual Doctor Visits, you can ask questions, get a diagnosis, or even get medication prescribed and have it sent to your pharmacy. Virtual Behavioral Health Visits may be best for initial evaluation, medication management, addiction, depression, stress or anxiety.

Stay healthy at home

UnitedHealthcare Healthy at Home provides you with the support you need to recover from hospital and skilled nursing facility stays. After you have been discharged, you are eligible to receive home-delivered meals, transportation to medical appointments and in-home personal care to assist with daily activities, all at no cost to you.
